Our Services
Probari helps nursing homes by utilizing a team of virtual nurses to take on complex tasks essential to resident care in nursing homes.
How We Help
The frontline staff have many responsibilities, including tasks that do not require the nurse to be in the facility. Reviewing hospital records, reading notes, piecing together treatment histories and chart reviews for quality improvement are time consuming. Our virtual support frees your nurses from excess paperwork allowing them to focus on day to day patient care.
The Probari Model
Probari reviews have been specifically designed to offload activities that are time-consuming, critical to operations, and refined over time to ensure that you get high-quality recommendations day after day. The program is easy to use, actually saving you time without distraction. Best of all, your team can choose the parts of the model that best fit your needs.

Admission Reviews
Transitions into facilities are often challenging due to either incomplete or too much information to process, which may impact resident care and safety. Probari’s team of virtual nurses reconcile and review information from the hospital and within the facility's medical record to create a checklist for the care team to quickly address issues.
Dashboards summarizing trends and aggregated data are accessible in real-time. This standardized offering has been rigorously tested in facilities and has shown to reduce readmissions to the hospital by 20% after the very first month.
Quality Reviews
Our Quality Reviews assess Quality Measures to provide actionable recommendations to address issues now and in the future, allowing facility leaders to dig deeper into issues. Using easy-to-navigate checklists, facilities are given clear action items to correct issues immediately and identify trends to prevent issues going forward.
Beyond our admission reviews, Probari has a library of additional review projects that are ready to be adapted for specific goals in any facility. Examples of specialty reviews can include lab tracking to close the gaps throughout a lab(s) journey and infection reviews to identify potential transmissions and mitigate current infections. These have been created from evidence-based practice and tested by our team to support care in the facility.

Education and Training
When facilities are truly going through top to bottom transition, there needs to be deliberate attention paid to the frontline staff that are caring for the patients. In truth, quality clinical data is only useful if the care team has the right training to make use of it at the bedside. For these facilities, we offer a bank of resources that takes a practical approach to teaching the basics, and our train-the-trainer model empowers the facility leadership to take the lead in the long term.
